Abstract:
The Mecklenburg County Community Resource Center (CRC) was created to provide integrated Health and Human Services (HHS) to residents through a coordinated delivery of HHS and other community-based programs. The CRC is the first of six planned, place-based service sites strategically located throughout the county. With the implementation of the CRC, customers no longer require visits to multiple locations to request assistance. Customers seeking services such as Child Support Enforcement, Food and Nutrition Services, Work First, Medicaid, Emergency Financial Services, Veterans Services, Women, Infants, and Children (WIC), Immunizations, Pregnancy Care Management and Care Coordination for Children (CC4C) can access these services at one location via an integrated service delivery model, focused on assisting individuals and families with moving forward into greater independence and well-being. In addition, customers may access some Community Partner organizations within the CRC as well. Enhanced customer experience is a primary focus of the service delivery model offered within the CRC. By offering an open and friendly environment, electronic check-in, expedited services for simple transactions, cross-service coordination, and access to community partners, the CRC provides an improved service delivery model for HHS customers which seeks to strengthen individuals and families, promote health and wellness, and build communities.
