Coordinated Care Collaborative

Abstract:
The San Bernardino County Department of Aging and Adult Services (DAAS) is tasked with serving seniors and adults with disabilities and assessing the need for in-home supportive assistance with Activities of Daily Living (ADLs). This population stood the risk of losing the benefits of care coordination. In an effort to sustain the improved quality of service post Coordinated Care initiative (CCI), San Bernardino County DAAS took the initiative to collaborate with two local managed care plans to keep care coordination going. This is to increase the likelihood that dual eligible beneficiaries will remain safely in their own homes and communities instead of moving out to receive costly institutional care. As part of the Coordinated Care Collaborative, DAAS renewed its Memoranda of Understanding (MOU) with the Inland Empire Health Plan (IEHP) and Molina Healthcare of California in January 2018 in order to continue sharing data; the MOUs are effective through December 2022. The Coordinated Care Collaborative enables the County and the managed care plans to improve outcomes for mutual clients that are enrolled in Cal MediConnect and have a higher risk of ill-health and more complex health care needs.
